[Osteonecrosis and HIV]. / Osteonecrosis y VIH.

An increase of the incidence of osteonecrosis in subjects infected with the HIV has been reported. It is unknown if osteonecrosis is an HIV infection consequence, or just the consequence of so many risk factors present among HIV infected subjects. We report five osteonecrosis cases found in our series of 534 HIV patients, and we review the related bibliography. All cases have some conventional risk factor different to the HIV or the antiviral treatment. We can't conclude antiviral treatments are not involved in osteonecrosis, but they have been the only risk factor in none of our patients.

En la literatura aparece un incremento de la incidencia de osteonecrosis de cabeza femoral entre sujetos infectados por el VIH, si bien se desconoce si ésto es consecuencia directa o indirecta de la enfermedad infecciosa o más bien se debe al acúmulo de factores que en estos pacientes se da por su pluripatología. En este trabajo describimos cinco casos de osteonecrosis ocurridos entre los 534 pacientes VIH vistos en nuestro hospital y repasamos la bibliografía relacionada. Todos los casos presentan alguno de los factores de riesgo convencional ya relacionados con el desarrollo de osteonecrosis. Si bien no puede descartarse el papel del tratamiento o del VIH en el desarrollo de osteonecrosis, no hemos detectado ningún caso entre nuestros pacientes en que éste haya sido el único factor (AU)

[Adult respiratory distress syndrome secondary to varicella pneumonia in an immunocompetent male]. / Síndrome de distress respiratorio del adulto secundario a neumonía varicelosa en varón inmunocompetente.

We present a case of varicellous pneumonia in a 40-year-old patient without immunitary compromise, who quickly evolved to respiratory distress in the adult and required sustained ventilatory support. We review the literature, with special mention to the indications of the antivirical treatment with Aciclovir.

[Leucocyte chemotaxis in patients treated with chronic haemodialysis (author's transl)]. / Quimiotaxis leucocitaria en enfermos tratados con hemodiálisis crónica.

Granulocytic and monocytic chemotaxis are studied by means of Boyden's modified method using as chemotactic agent human serum complement activated by Zymosan. The study has been made on healthy controls and a group of patients suffering from chronic renal failure treated with periodic haemodialysis. Observations were made before and after the dialysis session. In the patients with chronic renal failure is observed a decrease in leucocytic mobility of granulocytes and monocytes which give chemotactic granulocytic and monocytic indexes of 32.1 +/- 3.1 and 32.3 +/- 3.5 with relation to healthy controls with indexes of 46.7 +/- 8.4 and 37.0 +/- 4.3 and with significative differences between the measurements of p less than 0.001 and p less than 0.005 respectively. The parametres of polymorphonuclear and monocytic kinetics did not improve after the haemodialysis session. The possible mechanisms which produce such chemotactic leucocytic deficit in uraemics and the non-positive influence of the dialysis session on such deficit are hereby discussed.

[Bisalbuminemia and pseudobisalbuminemia: clinical significance (author's transl)]. / Bisalbuminemia y seudobisalbuminemia: significación clinica.

Two cases of bisalbuminaemia are hereby studied: a congenital case and an acquired or transitory case. In the first one, this hereditary disturbance is studied on four affected members of one Spanish family, being associated in two of them with multiple lipomatosis. This kind of association is considered as casual. As in most of the informed cases, ours belongs to the slow kind. The second case deals with a bisalbuminaemia of transitory character or pseudobisalbuminaemia which, appeared in a woman in the course of an acute pancreatitis of biliolithiasic origin, with a pancreatic pseudocyst and pleural effusion, the evolution in the inflamatory pancreatic process being advantageous in spite of the prsence of bisalbuminaemia. Briefly revised, are exposed the causes of this interesting and rare alteration in the electrophoretic fractioning of plasmatic proteins: congenital, and adquired causes due to administration of betalactamines and during the course of acute pancreatitis.
